If you use this online claim form and your claim is valid, you will receive the Software Benefit. If you want the Cash Payment Benefit instead of the Software Benefit, you must submit a mail-in claim form. You can receive the Cash Payment Benefit only if: (1) your purchase occurred before January 1, 2006; and (2) you submit approved proof of purchase. Click here to download the mail-in claim form. The deadline to submit a claim is July 23, 2010. Claims that were previously submitted do NOT need to be submitted again. All previously submitted claims will be processed by the claims administrator after Final Approval. If you have previously submitted a claim, you may not make an additional claim for the same drive.
